Our Nursing & Medication Assessment Process

A nursing & medication assessment with Axiom is a structured but personalized and holistic approach that provides a full picture of the client’s medical condition and readiness for treatment and recovery.

The Initial Consultation

The initial consultation is with the nurse meets with the client in person wherever possible. ( If not possible, then a virtual consultation will be arranged.) During this time, the nurse will gather background information, which includes a medical history, mental health diagnosis, any medications that the client is currently taking, and will touch on any concerns that the client may have.

The Physical Health Evaluation

This must be done in person. During this evaluation, vital signs are taken and a head-to-toe physical overview is conducted to detect any signs of withdrawal, medication side effects, or any co-existing physical or mental conditions that could impact recovery.

Medication Review

The nurse will carefully examine each prescription and any over-the-counter medication or supplement that the client is using. This will help to identify any drug interactions, duplication of medication, or incorrect dosages.
