Individualized Treatment Placement

Based on the evaluation and initial assessment of the patient, the Axiom Intervention and Case Management Services (AICMS) team can devise a plan in keeping with empirically derived data and American Society of Addiction Medicine (ASAM) criteria to connect your loved one with the best treatment services available. 

This can include liaising with treatment facilities, individualized supportive and clinical services, and on-going discharge plans.
